Employees needing to submit a paper version of the form can utilize the forms below:

  • Form 1 – This is the form containing the questions required by Part I of the University of Tennessee’s Conflict of Interest policy.  Please click here to access the form.

Once the employee has completed the form, it needs to be submitted to the Supervisor for approval and then route to the CBO’s office for review. For questions, please contact your campus business office.
